As the health care system becomes more consumer-driven, TransforMED, an $8 million practice redesign initiative of the American Academy of Family Physicians, will announce the launch of a demonstration project that will test a more patient-centered, consumer-friendly model of care in 36 medical practices across the nation.

This innovative practice redesign project, the result of several years of groundbreaking research, is a real-world test that holds the promise of changing the way primary care is delivered in America.
